Care pathway

Manufacturer direct means buying the medication from the drug company’s own pharmacy and paying cash, instead of going through insurance and a retail pharmacy.

The direct channels:

Why it matters: it is usually the cheapest cash route, and by a wide margin. The makers sell direct for a few hundred dollars a month against a list price over $1,000 — the same drug, from the same company, at a fraction of the published number.

The catches. It is cash-pay, so insurance doesn’t apply — if your plan covers the drug, a copay may still beat it. The cheapest advertised figures are usually limited-time offers for new patients on starting doses, and they expire. And some products are priced by dose, so what you pay rises as you titrate up.
